In the space below, please provide detailed information about the allegation you believe needs to come to the attention of the Compliance Officer of NTS Mortgage Income Fund, if your allegation is against the Compliance Officer this report will be forwarded to a member of the Board of Directors instead.
You do not have to identify yourself if you do not wish to, however, please understand that it may be difficult to conduct a thorough investigation without the opportunity to ask questions of the person making the allegation. If you do choose to maintain your anonymity, making sure you have provided very detailed information will assist us in our investigation. Whether or not you choose to remain anonymous NTS has a "No Retaliation Policy" that will protect you as long as this report is being submitted in good faith.
